Chapter 1 - The Weight of Two HeartbeatsThe sterile white walls of the downtown medical clinic seemed to blur as the rhythmic, galloping sound of a second heartbeat echoed through the small examination room. Lena Vance sat frozen on the paper-covered table, her breath catching in her throat. Her fingers gripped the edges of the cushion so tightly her knuckles turned white.

"Twins," the doctor murmured softly, a warm, professional smile breaking across her face as she pointed to two distinct, flickering pulses on the ultrasound screen. "Everything looks completely healthy, Mrs. Moretti—or should I say, Ms. Vance?"

Lena blinked back a sudden rush of hot tears. Twins. Not just one innocent life tethering her to a future she could no longer predict, but two. Two tiny, helpless souls who would inherit the legacy of Dante Moretti—the most feared, untouchable mafia kingpin in Chicago.

Just three weeks earlier, Lena had walked out of Dante’s private office with a single bag and a shattered heart, leaving behind the five-year marriage she had sacrificed everything to build. She had discovered Dante holding Sofia Vale against the mahogany desk where he commanded his underworld empire. She had listened to his excuses, swallowed the poison of his gaslighting, and walked away before the man who controlled half the Midwest could convince her that black was white.

Now, sitting in a quiet clinic miles away from her old penthouse, Lena realized the terrifying magnitude of her escape.

She could hide from Dante for a week, perhaps a month, but she could not hide forever. Sooner or later, the Chicago underworld would whisper, or a medical database would flag her insurance, and Dante would discover the truth. And when the king of Chicago realized his fugitive wife was carrying his heirs, the city would burn before he let them stay out of his reach.

Lena pulled her coat tight around her shoulders as she stepped out into the biting autumn wind. Her mind raced with options, none of them safe. She couldn't return to her mother’s house; Dante’s men had already flooded the neighborhood with flowers and inquiries disguised as well-wishers. She needed to vanish completely—not just from Dante’s sight, but from the grid entirely.

By evening, she had checked out of her temporary hotel room, destroyed her burner phone, and purchased a one-way ticket out of Illinois under a name she had borrowed from an old college textbook: Elena Vance. She packed her sketchbook, her grandmother’s necklace, the ultrasound prints tucked safely into her journal, and nothing else.
